H1B Checklist
Candidate Information
- Copy of passport (cover to cover) and I-94 (if applicable).
- Resume that describes:
- Education
- Work experience
- Project details and Special skills
- Copies of all degree certificates, i.e., Bachelors, Masters, etc.
- Copies of transcripts (marksheets) from all colleges for all degrees.
- Copies of all diploma or training course certificates.
- Results of any technical exams passed (if applicable).
- Documents proving completion of high school, i.e., secondary and higher secondary school exam results, school leaving certificates (if available), etc.
- Letters of professional references, recommendations, etc.
- Awards or other types of recognition (publications, etc.,if applicable).
- Marriage certificates (if spouse will apply for H-4).
- Evidence of name change (including maiden name, etc., if applicable).
- Copies of “Notice of Action” from Immigration and Naturalization Service for any previous petitions filed on behalf of the alien, including prior H=1, H-4, etc.
- F-1/students should provide I-120s and/or copies of Practical Training (EAD) documents, if applicable.
- If the candidate is already on H1-B Visa and is extending or moving from Company A to Company B, please provide a copy of W-2 from prior year and a copy of a recent pay stub, if employed.
- H-4 Documentation, USCIS approval notice for spouse/dependents status extension, if applicable.
- Company Profile (General background of the company, clients, product, services, etc.).
- Company Name, address, phone, fax, and authorized person name and title
- Net and gross income figures $$$
- Employer Tax ID and Business Address
- Number of employees (Staff)
- Description of product and services the Company provides
- Date company was founded
- Provide Company brochure, literature or information package
- Name/title of the Authorizing personnel for signatures.
- Job title and job description (education & experience) requirements for the H-1B position.
- Candidate’s salary and benefit package information.
- USCIS Filing Fee $ 320 +$500+$750 (under 25 employees) or 320+$500+$1500 (over 25 employees) payable to the US Department of Homeland Security. Add $1,000 for premium processing payable to the same. In some cases we may have to pay an additional $2,000 to USCIS if the total company employee number is 50+ and 50% of the sponsoring employer staff is on an H-1B, L-1/L-2 visa status.
- First time H1-B applicants may need a credential evaluation for an additional fee of $250-350 to be paid advance.
All documents/transcripts not in the English language must be accompanied by certified translations.

- Commercial Lease – Amendments to the Lease
- Stock Purchase Agreement-Investors Providing financing of $$ million

- Financial Proforma projections for one to three years
- Balance Sheet
- Income Statement
- Corporate Tax Returns or an Audited Financial Statement
- Payroll Register
- Marketing Brochures and Presentation Materials
